Before you start

You need:

an email address and phone number so we can contact you;
important medical information about your child(ren) that a driver would need to know in case of emergency; and
contact information for one emergency contact who has given you permission to share their details.

We collect and use personal information through this online form so we can ensure student safety, determine eligibility and provide student transportation services using a contractor. Non-identifying data and information may be used for statistics, reporting and service delivery review.
